STEPS TO BECOME A RESIDENT OF THIS COMMUNITY:

Once you or your loved one decides to join the Community of Shepherd of God, various forms and documents that need to be completed are available to be printed from this website: the "Physician's Report, Admission Agreement, and Resident's Rights" or come into our office for copies.

The perspective resident will need to see a physician so the physician can complete the state licensing paperwork as well as get a tuberculosis clearance.  You can print the physicians report LIC 602 below and take it to your physician for completion which can be faxed by the physician to Shepherd of God, mailed or you may bring it in person. Our Administration staff will then need to review this report once it is completed.

If you wish to review and complete our Admission Agreement, you may print it, sign it, and mail it in or bring it to Shepherd of God in person.
